Give the clinic profile slug from its page address and identify one affected field. Do not include patient records, personal health information or credentials.

Submitting a report does not change public visibility. An accountable platform administrator can separately apply or release a safety hold, accept the report, reject it or record that no change is needed. Proposed values remain private until they pass a separate publication decision.

We do not promise that an external phone number, website or destination is live merely because its format and source checks passed.
